    A long-term clinical study on oral terbinafine combined with topical bifonazole cream for tinea pedis treatment

    Objective To find the best schedule for tinea pedis treatment.Methods A total of 250 patients with tinea pedis were divided into five groups: group A (oral terbinafine combined with 1% topical bifonazole cream for 1 week),group B (oral terbinafine for 1 week combined with 1% topical bifonazole cream for 2 weeks),group C (oral terbinafine for 2 weeks combined with 1% topical bifonazole cream for 1 week),group D (oral terbinafine combined with 1% topical bifonazole cream for 2 weeks),and group E (only 1% topical bifonazole cream for 4 weeks).Efficacy and relapse rates were analysed at the end point,week 4,24,40,56 and week 72.Results By the end of week 24,40,56 and week 72,clinical effective rate in group A,B,C and D showed statistical difference with that in group E,while group D showed statistical difference with group A and B by the end of week 40,56 and week 72.At week 24,40,56 and week 72,microbiological cure rate in group A,B,C and D showed statistical difference with that in group E,while group D showed statistical difference with group A and B at week 56 and 72.At week 40,relapse rates in group A,B,C and D were statistically different from that in group E,while at week 56 and 72,group D showed statistical difference with group A and B.Conclusions Two-week treatment with oral terbinafine and 1% topical bifonazole cream has the best therapeutic effect and lowest relapse rate.

    Epidemiology of aspergillosis in mainland China

    Objective To understand the epidemiology of aspergillosis and the status of diagnosis and treatment in mainland China.Methods Literatures from January 1991 to October 2009 were collected by CNKI and Pubmed and statistically analyzed.Results A total of 1 447 cases were included in our study,in which 481 cases had microbiological evidences of aspergillus,258 cases did species identification with 153 strains of A.fumigatus.Pulmonary infection was presented in 1 047 cases.Etiological diagonosis depended on direct microspcopy and culture,while species identification relied on morphology except 4 cases with identification of molecular biology.G-test and GM-test were performed in few cases and in vitro susceptibility test was fewer.Only 2 strains of A.flavus were resistant to AMB and 2 strains of A.fumigatus were resistant to multidrugs.Itraconazole pretended to be the first choice for invasive aspergillosis.Corticosteroid alone or combined with itraconazole was applied to allergic bronchopulmonary aspergillosis.Surgical excision was recommended for chronic,saprophytic,nasal and paranasal sinus aspergillosis.Conclusions Trend of aspergillosis in mainland China was in concordance with the world.Mobidity of non-fumigatus aspergillosis increased,especially pulmonary involvements.Morphological and molecular biological identification are important for accurate and prompt diagnosis.In vitro susceptibility test is helpful for clinical treatment.

    Caspase recruitment domain-containing protein 9 (CARD9) is an adaptor molecule that is critical for NF-κB activation,and forms a complex with B-cell lymphoma 10 (BCL10) and mucosa-associated lymphoid tissue lymphoma translocation gene 1 (MALT1) that mediates C-type lectin receptors (CLRs)-triggered intracellular signaling during antifungal immunity.In the present study,we investigated the functions of polymorphonuclear neutrophils (PMNs) from phaeohyphomycosis patients with CARD9 deficiencies against Candida albicans (C.albicans).PMNs were isolated from patients and healthy blood donors and subsequently challenged with C.albicans.By calculating colony-forming units and MTT assay,the killing of C.albicans by CARD9-deficient neutrophils was detected,and the ROS production and phagocytotic ability of CARD9-deficient neutrophils was assayed by Flow CytoMetry (FCM).The phagolysosome ultrastructure and the secretion of IL-8 were analyzed separately by EM and CBA kit.As a result,we demonstrated that,compared with healthy donors,CARD9-deficient PMNs exhibited defects in C.albicans killing and IL-8 productions,which can be rescued in the presence of serum;however,the CARD9-deficient PMNs exhibited normal reactive oxygen species generation,phagocytotic ability and phagolysosome ultrastructure.In conclusion,our results indicate that CARD9 is indispensable for C.albicans killing by PMNs,and serum-opsonization acts as a CARD9-independent way,which could be a promising immunotherapy in the future.

    Invasive aspergillosis (IA) is a serious opportunistic infection that mainly been found in individuals with a compromised immune system. As Aspergillus species worldwide and the increasing number of immunocompromised hosts, IA brings a heavy burden to our country. In addition, early diagnosis is difficult and effective treatments are lacking, which leaded to the high mortality of IA (39%~100%). Currently, further nationwide epidemiological research of IA is needed in China.

    Efficacy and safety of Meifute® antifungal solution in the treatment of tinea pedis cases from troops

    Objective To evaluate efficacy and safety of Meifute® antifungal solution in treating tinea pedis cases from troops.Methods Generally,a single-session soak in Meifute® antifungal solution for up to120-minute long was the suggested treatment of tinea pedis.As for some tinea pedis cases with hyperkeratosis or blisters,repeated treatment of one or two sessions at 14 day intervals might be recommended.Effect and safety evaluation was performed at week one (V1),week two (V2),and week four (V3) after initial treatment.Results Finally there were 206 cases of tinea pedis being enrolled into this study,including 104 cases of group Honour Guard (i.e.group H.G.)and 102 cases of group Armed Police (i.e.group A.P.).At week four (V3),overall effective rate was 95.6% (197/206),the cure rate was 78.2% (161/206),overall fungal clearance rate was 95.1%.Group H.G.had an effective rate of 96.1% (100/104) and a cure rate of 81.7% (85/104).Group A.P.got an effective rate of 95.1% (97/102) and a cure rate of 74.5% (76/102).There were only several cases of local side effects including mild pain (51/212) that could relieved once soak finished,and local mild edema (12/212)that self-subsided during 2 hours after soak.Follow-up of 6 months later revealed a recurrence rate of 8.97% (14/156).The recurrence rates of Group H.G.and Group A.P.were 3.06% (3/98) and 18.97% (11/58),respectively.The comprehensive satisfactory rate of patient reached up to 95.32%.Conclusion Meifute® antifungal solution has showed good efficacy and is well tolerated in treating tinea pedis.It is especially recommended to be used in the patients who requiring a fastly and rapidly responsed therapeutic method to relieve their symptons and signs of tinea pedis within a short duration.

    Objective To report 80 cases of candida vulvovaginitis received adjuvant management.Methods The adjuvant management is including methods of psychological counseling, guidance, medication and health education.Results Seventy-two cases (90.00%) recovered after a course of treatment, 6 cases (7.50%) were cured after two courses and 2 cases (2.50%) changed into the refractory candida vulvovaginitis.Conclusion Psychological counseling, guidance, medication, skin care and health education are benefit for the treatment of patients with candida vulvovaginitis.

    Sertaconazole nitrate is a new broad-spectrum, efficient topical azole antifungal.This paper summarizes its pharmacological action, anti-inflammatory, antipruritic action, antifungal activity, the latest clinical efficacy and safety evaluation in the treatment of dermatophytoses, superficial candidiasis, seborrheic dermatitis and atopic dermatitis etc.Compared with miconazole, clotrimazole, terbinafine etc, sertaconazole cream can provide a better efficacy and tolerability in the treatment of skin tinea and superficial Candidal infections.Its antipruritic action and once-daily application in the light of its pharmacokinetics increase patient adherence and the cure rate.A topical microemulsion-based hydrogel has been developed to enhance permeation of sertaconazole for effective eradication of cutaneous fungal infection.The latest clinical studies suggest that sertaconazole cream may be an excellent alternative therapeutic modality for treating seborrheic dermatitis and atopic dermatitis.

    Objective To investigate anti-adhesion effect of Butyl alcohol extract of BaiTouWeng decoction (BAEB)on Candida albicans.Methods Agar plate assay was used to determine CFU of Candida albicans for 2h and 4h.XTT reduction assay was applied to evaluate the adherent Candida albicans merabolic activity.Test tube assay was utilized to evaluate flocculation.The water-hydrocarbon two-phase assay was employed to measure the cell surface hydrophobicity of Candida albicans.Scanning electron microscope(SEM)and fluorescein diacetate(FDA)staining was used to observe the morphological changes of adherent Candida albicans.qRT-PCR was adopted to inspect the expression of adhesion-related genes such as INT1,ALS3,MNT2,ALA1,ALS1,ALS5,HWP1 and CSH1.Results 512,1 024 μg/mL BAEB could effectively inhibit adherence of Candida albicans.Flocculation was delayed by 512,1 024 μg/mL BAEB.The cell surface hydrophobicity in the BAEB treated groups was lower than that of the control group.The expression of ALS1,CSH1,ALA1,ALS3,INT1,HWP1 were downregulated dramatically after BAEB treatment,but ALS5 and MNT2 were unchanged.Conclusion BAEB could effectively inhibit adherence of Candida albicans isolated from VVC by regulating adhesion-associated genes.

    Objectives To investigate the in vitro antifungal susceptibility of FK-506 combined with antifungals to Candida spp.and dermatophytes. Methods The minimal inhibitory concentrations (MICs) of Candida spp.against ketoconazole and dermatophytes against terbinafine were assayed using CLSI M27-A3 and M38-A2 microdilution method,respectively.By using broth microdilution checkerboard technique system,we evaluated the combined antifungal activity of tacrolimus with ketoconazole or terbinafine against Candida spp.and dermatophytes,respectively. Results The MIC50 and MIC90 of ketoconazole against Candida spp.were 4 μg/mL and 8 μg/mL,respectively.The MIC50 and MIC90 of terbinafine against dermatophytes were 0.06 μg/mL and 0.125 μg/mL,respectively.The result showed that the combination of tacrolimus and Ketoconazole or Terbinafine had synergistic inhibitory activity against Candida spp.(53.5%,n=15) and dermatophytes (46.7%,n=15). Conclusion The MICs of antifungals can be reduced partly when combined with low concentration of FK-506 against Candida spp.and dermatophytes,and no antagonism was found.

    Innate immunity is the first line of the host immune system fighting against invading pathogens.Pattern recognition receptors (PRRs) are responsible for recognition of pathogens and their components.And the nucleotide oligomerization domain (Nod)-like receptors (NLRs) are intracellular PRRs.In recognition of pathogens infection, some NLRs including NLRP3, NLRP1 and NLRC4 form large protein complexes called inflammasomes, which mediate maturation of pro-inflammatory cytokines IL-1β and IL-18.Inflammasomes are important in anti-fungal immunity.Cryptococcus neoformans is an opportunistic fungal pathogen, mainly infecting immunocompromised patients and causing high mortality.Here we review recent progresses on host innate immune response against Cryptococcus neoformans.

    Objective To find a rapid, effective and easy-to-perform DNA extraction protocol from Sporothrix globosa.Methods Four methods, such as CTAB-extraction, guanidine thiocyanate boiling-extraction, alkaline lysis-extraction, DNA extraction kit, were compared based on Sporothrix globosa isolates from different regions.The purity and yield of the DNA were determined by ultraviolet absorption spectrum.The DNA quality was evaluated with agarose gel electrophoresis and PCR amplification.Results It showed that all methods could get DNA, but alkaline lysis-extraction was better than others for the purity and yield of the DNA.Based on degenerated primers of fragments of the calmodulin genes for PCR, we found all samples showed bright stripe.Conclusion Alkaline lysis-extraction is the most suitable method for extracting high quality DNA from Sporothrix globosa.

    Objectives To investigate the in vitro susceptibility testing of terbinafine against 138 strains of dermatophytes.Methods CLSI M38-A2 guideline was applied.The media containing the fungi in the concentration of(0.4-5)×104 CFU/mL were incubated at 30 for 7 days.The MIC endpoint was determined as the first concentration of antifungal agent that reached 100% inhibition of growth compared with the growth control.The MEC endpoint was determined as the first concentration of antifungal agent that reached 50% inhibition of growth compared with the growth control.Results The tested 138 strains of Dermatophytes were susceptible to terbinafine,MEC range:0.008-0.125 μg/mL,MIC 0.008-0.25μg/mL.There was no statistical differences between Beijia terbinafine and imported terbinafine(P>0.05).Conclusion Beija Terbinafine is active aganist Dermatophytes.It can be an effective choice in superficial mycosis treatment.

    Objective To design targeted gold nanoparticles (GNP) conjugated with anti-Cryptococcus neoformans antibody,to explore targeted gold nanoparticles-mediated photothermal therapy of Cryptococcus neoformans in vitro.Methods Gold nanoparticles were synthesized according to seed-mediated template-assisted protocol,conjugated with anti-Cryptococcus neoformans antibody(Ab).Cryptococcus neoformans were incubated with targeted gold nanoparticles,then exposed to near-infrared (NIR) light.Cell viability was evaluated.Results Targeted gold nanoparticles were synthesized successfully and can be used to selectively target Cryptococcus neoformans cells and following NIR laser exposure,can be used to significantly reduce cell viability.Conclusions These results demongstrated that Ab-mediated immunity gold nanoparticles can enhance NIR photothermal treatments effect on Cryptococcus neoformans.

    Clinical and etiologic features in 52 oral candidiasis patients with HIV/AIDS

    Objective Oral candidiasis is considered one of the most common oral diseases of Human Immunodeficiency Virus /Acquired Immune Deficiency Syndrome patients.Understanding the clinical and etiologic features of the disease plays a vital role in its diagnosis and use of medication.Methods Mucosal swab samples were collected from 70 HIV/AIDS patients with oral candidiasis.And yeast colonies were developed and identified by CHROMagar Candida.Results There were 52 cases of oral candidiasis with 70 HIV/AIDS patients,mainly manifested in Pseudomembranous and erythema types,of which Pseudomembranous is the most common.The detection rate of Candida albicans was the highest (60.32%) in patients with oral candidiasis HIV/AIDS.Other identified Candida species were C.tropicalis (19.05%),C.glabrata (12.70%) and C.crusei (7.94%).There were 9 cases of oral candidiasisin of HIV/AIDS patients with mixed infection and 2 cases with three kinds of pathogenic bacterias.Conclusion The Clinical manifestation of HIV/AIDS patients with oral candidiasis is complex and concurrent.However,diagnosing based merely on its clinical appearance of the disease is insufficient.Therefore,accurate classification of pathogenic bacterias can guide the usage of clinical medication and reduce the production of drug resistant bacterias.

    Objective To obtain a RNA-interference method mediated by Agrobacterium tumefaciens applicable for gene research on filamentous fungi.Methods Gene recombination and interference system selection were used.Results The recombinant vector PCB309-pfgrt was obtained and used in interfering the expression level of two-component DRK1 gene successfully.Conclusions This method optimized the transformation system,solving the problem of high efficient RNA interference vector construction,which has wide application prospect in genetic research in filamentous fungi.

    Candida albicans is one of the most common fungal pathogens of humans that colonize the skin and mucosal surfaces of the healthy hosts. Candida albicans can cause the widespread infection in the skin mucous membrane and viscera of the hosts.This microorganism is the single cell pseudohyphae yeast,which can grow as both of yeast and filamentous forms in the host,and viscerarmed fungal dimorphism or,more precisely,fungal polymorphism.The relative attributes of the yeast and filamentous forms of Candida albicans,during the colonization of skin and mucosa,and later in the invasion of the blood stream and deep tissues,had long been debated.The pathogenesis of Candida albicans had been attributed to multi-factors,including adherence to host cells,secretion of degradative enzymes,morphology switching,successful biofilm formation,quorum sensing and so on.In this paper,the recent research progress in the pathogenesis of Candida albicans was reviewed.

    Illegible sporotrichosis on the face

    To explore the nonspecific clinical manifestations of sporotrichosis. Sporotrichosis is a deep fungal disease with common cutaneous manifestations as chronic granuloma, ulcer and nodular lesions. Part of patients may not show the specific clinical manifestations becarse of miscellaneous reasons. This situation above increase the difficulty of final diagnosis to some dermatologist with little related clinical experience. The eight patients of sporotrichosis to be discussed in this paper didn't have the same clinical manifestation as those mentioned characteristic in a previous articles.This situation suggestes that the definite diagnosis should be closely according to the medical history, the living environment, and even the process of diagnosis and treatment.

    Preliminary screening active constituent against Candida albicans resistant to fluconazole from Rubus chingii Hu fruits

    Objective To compare three methods (cold-soak method, ultrasonic wave method and hot reflux method) of extracting antifungal constituents from Rubus chingii Hu fruits and preliminary screen active part of the extracts.Methods The three extractive methods were evaluated through antifungal activity in vitro of extracts from Rubus chingii Hu fruits combining with fluconazole against durg-resistant Candida albicans and extraction rate. The compositions of these extracts were separated with petroleum ether, chloroform, ethyl acetate, n-butyl alcohol and water. Screening out the active part were performed through evaluating antifungal activity in vitro of parts from the extracts of Rubus chingii Hu fruits combining with fluconazole against durg-resistant Candida albicans.Results The extraction rate of the Cold-soak method, ultrasonic wave method and hot reflux method was 21.0%, 16.8% and 12.8%. The FICIs of the extracts using three methods combining with fluconazole against Candida albicans 100 were 0.035 2, 0.032 2 and 0.046 9, and 0.033 2, 0.031 7 and 0.039 1 against Candida albicans 103. The FICIs of petroleum ether, chloroform, ethyl acetate, n-butyl alcohol and water part combining with fluconazole against Candida albicans 100 were >1, 0.502 0, 0.507 8, 0.033 2 and >1, and >1, 0.531 3, 0.507 8, 0.033 2 and >1 against Candida albicans 103.Conclusions Ultrasonic wave method is simple, reliable and efficient. Compared with the other methods, it is the best method of extracting antifungal constituents from Rubus chingii Hu fruits. The active part of the extracts from Rubus chingii Hu fruits is the n-butyl alcohol part.

    Antifungal susceptibility of 30 clinical isolates of Rhodotorula spp. and literature review

    Objective To test and analysis the antifungal activity of 5 drugs against clinical isolates of Rhodotorula spp. in vitro, and review the drug susceptibility data of another 249 strains of Rhodotorula spp..Methods Rhodotorula spp. were isolated and identified from clinical fungi bank of our hospital, susceptibility profile of these strains were tested with ATB Fungus3. A systematic MEDLINE search of English literature was performed to identify articles on Rhodotorula antifungal susceptibility in the past 20 years.Results Our results show that itraconazole, fluconazole and voriconazole are almost inactive against all of Rhodotorula spp.in vitro, Amphotericin B and flucytosine exhibit good antifungal activity,which is consistent with literature review.Conclusions The results show that, commonly used azole antifungal agents are inactive against Rhodotorula spp..Amphotericin B and Flucytosine are both active in vitro, being rational choice for empirical treatment.

    Construction of MYC-tagged Sch9p in Candida albicans by using of long primers PCR amplification

    Objective To construct the MYC-tagged Sch9p in Candida albicans.Methods Using a pair of the long primers to amplify sequences containing the MYC tag and the ARG4 selection markers from the plasmid pFA-ARG4-MYC.The amplified plasmid sequences were transformed into the C-terminus of SCH9 open reading frame (ORF) in C.albicans SN152 by homologous recombination.The positive colonies were selected in the SC-Leu- selective solid culture medium.Afterwards,the positive colonies with correct integration were confirmed by PCR of genomic DNA.Finally,these positive transfectants were examined by time-growth curve testing,drug sensitivity spot assays,and mycelium inducing experiments.Results Strains with MYC-tagged Sch9p which had normal phenotypes were selected.Conclusions The MYC-tagged Sch9p of C.albicans strains can be constructed correctly by using of long primers to amplify the plasmid sequences containing the MYC tag and ARG4 selection markers which were integrated into the C-terminus of SCH9 ORF in C.albicans by homologous recombination.
