    Efficacy and safety of Meifute® antifungal solution combined with Feijia® antifungal solution in the treatment of onychomycosis

    Objective To evaluate the efficacy and safety of Meifute® antifungal solution combined with Feijia® antifungal solution in the treatment of onychomycosis. Methods From December 2020 to November 2022, 120 patients were enrolled in Fumei Outpatient Departments for this study. First of all, a single-session soak in Meifute® antifungal solution was used to treat tinea pedis; and then, diseased nails were treated with Feijia® antifungal solution once a week for 28 weeks routinely; finally, another time of Meifute® antifungal solution treatment was given to consolidate the effect. Efficacy and safety evaluation was performed after treatment, and a questionnaire survey of treatment satisfaction was carried out after the completion of the treatment course. Results Finally 112 patients completed the study and 588 nails were included. Efficacy and satisfaction:overall effective rate was 58.0% (341/588), the cure rate was 20.6% (121/588), and the comprehensive satisfactory rate was 93.8% (105/112). The efficacy of the first toenail (52.9%) was lower than that of other toenails (60.7%). And there was no significant difference in the clinical response rate between DLSO (56.6%) and TDO (61.4%). Safety:The adverse reactions were all local symptoms, such as subungual hemorrhage (2 cases), exfoliation of epidermis around nails (4 cases), mild redness and swelling of skin (6 cases), mild to moderate burning or tingling (16 cases) after soak. All patients could tolerate the process without serious adverse reactions. Conclusion The use of Meifute® antifungal solution combined with Feijia® antifungal solution is simple, effective, safe, and highly compliant in the treatment of onychomycosis, so it can be regarded as a clinical alternative treatment for onychomycosis.

    Objective Two in vitro drug sensitivity test methods, ager plate diffusion method (punch method) and microliquid-based dilution method, were used to observe the in vitro antibacterial activity of Medi hair spray dressing and its main components(Microbial antimicrobial peptide) against common Malassezia on the surface of seborrheic dermatitis lesions of the head. Methods Medi Hair Spray Dressing with different concentration, microbial antimicrobial peptide were used as the experimental group, and Caile lotion (concentration 2% ketoconazole lotion), blank solvent of Medi Hair Spray Dressing were used as the control group,and the diameters of the inhibition zone were compared in each group. The inhibitory effect on Malassezia in vitro was tested by agar plate diffusion method (punch method) and microdilution method, and the MIC (minimum inhibitory concentration) values were also determined. Results Medi Hair Spray Dressing and microbial antimicrobial peptide inhibited Malassezia to varying degrees in vitro,and the effect was positively correlated with drug concentration. Medi Hair Spray Dressing MIC was 0.977 μL/mL;microbial antimicrobial peptide MIC was 3.907 μL/mL. Conclusion Medi Hair Spray Dressing is safe,and has a better inhibition of Malassezia effect.

    Objective To systematically evaluate the efficacy and safety of trimethoprim-sulfamethoxazole (TMP/SMZ) combined with caspofungin in the treatment of Pneumocystis jirovecii pneumonia (PJP). Methods PubMed, Embase, Cochrane library, CBM, CNKI, Wanfang, VIP were searched for clinical randomized controlled trials of TMP/SMZ combined with caspofungin in the treatment of PJP, and Meta-analysis was performed. The outcome indicators were total effective rate, incidence of adverse reactions Statistical analysis was performed using RevMan 5.4 software. Results A total of 20 studies were included, involving 1 802 PJP patients. Meta-analysis results showed that compared with TMP/SMZ monotherapy, the effective rate of TMP/SMZ combined with caspofungin in the treatment of PJP was significantly improved, [RR =1.29, 95% CI (1.22,1.37),P< 0.00001], and no Increase the incidence of adverse drug reactions [RR =0.91,95% CI(0.79, 1.05),P=0.21>0.05]. Conclusion The effect of TMP/SMZ combined with caspofungin in the treatment of PJP is better than that of TMP/SMZ monotherapy, without increasing adverse drug reactions, and it is recommended for clinical use.

    Objective To analyze epidemiological and clinical characteristics of infections caused by Rhodotorula mucilaginosa, and to provide a scientific basis for improving the clinical diagnosis and treatment of R. mucilaginosa infections. Methods Using the methods of literature review and meta-analysis to analyze retrospectively the information of patient's nationality, sex and aged composition, risk factors, onset organs, diagnostic and therapeutic methods and prognosis of R. mucilaginosa worldwide in the past two decades. Results A total of 45 related literatures were included, including 60 cases of patients infected by R. mucilaginosa. R. mucilaginosa infections was most common in Europe. The gender ratio of the infection host was 1.68:1, predominantly in people aged 0-10 years and 40-50 years, and the blood system was the most commonly affected site. Most patients had underline diseases or other risk factors such as trauma and surgery, and there was no early specific symptom with chief complaints of fever. Diagnosis of R. mucilaginosa infections mainly relied on culture. The mortality rate was as high as 12.5%, except in patients with unknown prognosis. Conclusion The incidence of R. mucilaginosa infections has increased recently worldwide. The most commonly affected site was blood system, and the diagnosis mainly depends on culture. Strengthening the in vitro antifungal susceptibility monitoring and the studying of epidemiology for R. mucilaginosa are benefical to improve the clinical diagnosis and prognosis of R. mucilaginosa infections.

    Clinical characteristics of 20 cases of PJP and the value of NGS in the diagnosis of PJP

    Objective To analyze the clinical features and high-risk factors of Pneumocystis jirovecii pneumonia (PJP), explore the value of metagenomic sequencing (mNGS) in the diagnosis of PJP, and seek possible prognostic indicators in PJP patients. Methods From April 2020 to December 2021, the clinical data of 20 PJP patients admitted to our department were collected, and their clinical characteristics were analyzed. The differences in laboratory indexes before and after treatment and the value of different detection methods in diagnosis were compared. Results Pneumocystis infection was detected by NGS in BALF of 20 patients, meanwhile, the positive rate of blood G test was 90%. However, neither smears nor cultures of the pathogen showed signs of pneumocystis infection. After anti-PJP treatment, 14 cases were clinically relieved, and the difference in peripheral blood lymphocyte. Conclusion Compared with the sensitivity of conventional etiological smears and cultures, mNGS showed better value in the diagnosis of PJP. NGS could identify pathogens with high sensitivity. In addition, peripheral blood lymphocytes were positively correlated with disease recovery and could be used as one of the laboratory markers of prognosis in PJP patients.

    Diagnostic value of quantitative real-time PCR in bronchoalveolar lavage fluid for HIV-negative talaromycosis marneffei

    Objective To determine the loads of Talaromyces marneffei (TM) in bronchoalveolar lavage fluid (BALF) of the patients with non-HIV-infected talaromycosis marneffei (TSM) by quantitative real-time PCR (qPCR) and to evaluate the diagnostic value of it. Methods A total of 17 cases of non-HIV-infected TSM patients with pulmonary infection symptoms and 33 cases of patients with other fungal infections in the lung or non-infected patients were collected from The First Affiliated Hospital of Guangxi Medical University from September 2019 to June 2022. Samples of blood and BALF were collected from every patient at the same time. Extracting DNA from their BALF and serum, and then using qPCR to measure the loads of TM in these samples. Fungal culture of BALF was performed at the same time. Results In 17 cases of TSM, only 11.8% (2/17) of these cases were positive for qPCR measured in serum, but the positive rate of BALF qPCR was 90.0% (9/10) in patients with positive BALF fungal culture. Besides, there were 2 cases with positive BALF qPCR test results but with negative fungal culture of TM using the same sample. All the serum and BALF qPCR of other fungal infections in the lung or non-infected patients in this study were negative. In the TSM group, the median Cq value for BALF qPCR detection was 31.40 cycles (range 20.71-35.59). The qPCR showed a substantial agreement with the gold standard (kappa:0.708) and superiority to the fungal culture (kappa: 0.653) and directly microscopy-examined (kappa: 0.220). Conclusion This study showed that using qPCR to measure the loads of TM in BALF could have a high value in the early diagnosis of non-HIV-infected TSM patients with pulmonary infection symptoms.

    Objective To collect the fungal bloodstream infection data from the Sichuan Provincial Center of China Fungal Disease Surveillance System, to analyze the distribution characteristics and the susceptibility to antifungal drugs of pathogens, and to provide epidemiological data for fungal bloodstream infections in Sichuan Province. Methods The data of outpatient, emergency, and inpatient fungal bloodstream infections in 28 hospitals from January 1, 2019, to December 31, 2021, were studied. The clinical and laboratory data of fungal bloodstream infection were analyzed using Whonet 5.6 and Microsoft Excle pivot table. information. Results From January 1, 2019, to December 31, 2021, Candida accounted for 88.8% (1083/1 220) of the 1 220 fungal strains was reported by 28 hospitals. Candida albicans was the main species, accounting for 35.3% (431/1 220).and Cryptococcus neoformans being fifth in the total number of isolates, accounting for 8.3% (101/1 220). Males accounted for 56.6% (690/1 220), and female patients were less (43.4%, 530/1 220); 79.8% cases were middle-aged and elderly patients (>46 years old) and 34.8% cases were from the intensive care unit. C. albicans had the highest sensitivity to fluconazole (85.7%); resistance rates of C.tropicalis to fluconazole and voriconazole were 43.4% and 43.3%, respectively. by C. albicans isolates were dominated in Most hospitals, however, C. parapsilosis was mostly isolated in children's specialized hospitals. Conclusion The pathogens of fungal bloodstream infection and their antifungal drug susceptibility have regional characteristics. Accurately grasping the epidemiological data in the region can provide a basis for the clinical selection of antifungal drugs.

    Study on the effect of fluconazole with subinhibitory concentration on the virulence of Candida glabrata based on the Galleria mellonella infection model

    Objective Based on the Galleria mellonella infection model, the effect of sub-MIC fluconazole on the virulence of Candida glabrata was investigated. Methods Non repetitive Candida glabrata strains clinically isolated from Qinghai Provincial People’s Hospital from April 2021 to October 2022 were collected, and the minimum inhibitory concentration (MIC) of fluconazole against Candida glabrata was determined by ATB method. The virulence of Candida glabrata was tested by the Galleria mellonella infection model, and the success of the model was evaluated by pathological and pathogenic methods. Effects of puncture and different sub-MIC FLUs on healthy Galleria mellonella: the control group did not do any treatment, and the experimental group was divided into three groups, namely PBS group (10 μL PBS), FLU subgroup(inject 10 μL fluconazole solution respectively, making the FLU concentration in the body of infected Galleria mellonella 0.5 mg/kg, 1 mg/kg, 2 mg/kg, 4 mg/kg, 8 mg/kg, and 16 mg/kg) and puncture group (micro syringe puncture but no injection). Use 2.50×106 yeasts /larva infected with Galleria mellonella, after 2 h, the control group was injected with 10 μL PBS, the experimental group was injected with different concentrations of FLU working solution, observing the therapeutic effect on the Galleria mellonella infected by Candida glabrata. Results The MIC of fluconazole to 9 clinically isolated Candida glabrata was 8 mg/L~32 mg/L, and that of Candida glabrata ATCC2001 was 32 mg/L. Yeast-like spores were detected in the pathological section of the intestinal tissue of the Galleria mellonell in the experimental group, and Candida glabra was cultured in the intestinal tissue.No fungus was found in the pathological section and fungal culture of the intestinal tissue in the control group. Puncture could cause damage to the Galleria mellonella, but the mortality was less than 10%. 1/2 MIC fluconazole could improve the survival rate of Candida glabrata infected Galleria mellonella with statistical significance(P<0.05),while 1/4 MIC, 1/8 MIC,1/16 MIC fluconazole had no significant intervention effect on the survival rates of Candida glabrata infected Galleria mellonella compared with PBS and the difference was not statistically significant (P>0.05). Conclusion 1/2 MIC fluconazole had inhibitory effect on the virulence of Candida glabrata, but the concentration below 1/2 MIC had no obvious protective effect.

    Distribution of pathogenic yeast species of vulvovaginitis and its epidemiological characteristics

    Objective To investigate the epidemiological characteristics of vulvovaginitis caused by different yeast spp. Methods Totally 1 083 yeast isolates were collected from patients with vulvovaginal candidiasis (VVC). These isolates were identified to the species level according to the genes of 26S rRNA D1/D2 domain. The clinical characteristics VVC cases caused by these different species were analyzed. Results The results showed that Candida albicans remain the prodominent species(71.38%) causing VVC, while Non-candida albicans (NAC) account for 28.62% among which C.glabrata account for 19.02%. The risk factors included 20-40 years of age, being married,and pregnancy. NACs were more often isolated in pregnant women. The incidence of recurrent vaginal candidiasis caused by non-albicans Candida were higher. Conclusion Routine culture of secretion from VVC patients, molecular identification to species level of VVC causative agents, and identification of non-albicans Candida species would help clinicians to explore an effective therapy for VVC.

    A study on the distribution and the biofilm formation related genes of Candida parapsilosis complex clinically isolated in Guizhou area

    Objective The distribution of infection and drug susceptibility characteristics of Candida parapsilosis complex strains were studied in Guizhou. Detection of the biofilm formation capacity of strains and the expression level of its CDC28 gene were performed. Methods The BACTECTM FX and Phoenix-100 instrument were used to identify the strains isolated in there tertiary first class hospitals in Guizhou from January 2017 to December 2019. The Candida parapsilosis complex group was identified by gene sequencing. ATB FUNGUS 3 kit was used to detect drug susceptibility of strains. The Candida parapsilosis biofilms formation capacitywas detected, and the expression levels of their CDC28 genes were detected using qRT-PCR. Results A total of 163 strains of Candida parapsilosis complex were isolated and identified, of which Candida parapsilosis type I, II and III accounted for 73.0%, 6.1% and 20.9% respectively. Urine and blood specimens got the most isolated strains (both 50 strains). Among the ioslates from infected bloodstream, type I, II and III accounted for 54.0%, 10.0% and 36.0% respectively. Three genotype strains were not significantly resistant to antifungal drugs. Among the 17 clinical strains, 16 (94.1%) had biofilm formation ability, of which 2, 4 and 10 strains had strong, general and weak biofilm formation capacity. CDC28 expressed upregulation in 2 strains with strong and 3 strains with general ability. Conclusion The isolated Candida parapsilosis complex was dominated by type I, and the three genotype strains were not obviously resistant to antifungal drugs. Different Candida parapsilosis showed different biofilm formation capacity, and the CDC28 gene might be associated with biofilm formation.

    Analysis of clinical features and death-related risk factors of Candida bloodstream infection

    Objective To explore the clinical features of Candida bloodstream infection. Methods The clinical data of 62 patients diagnosed with Candida bloodstream infection in our hospital from January 2017 to December 2021 were collected, and the distribution of pathogen, clinical characteristics and risk factors of death were analyzed. Results A total of 65 strains of fungi were isolated from 62 patients with Candida bloodstream infection, mainly distributed in the intensive care unit (33 strains), urology (7 strains) and hematology (6 strains) departments; the strains were Candida albicans (29.2%), Candida glabrata (26.2%), Candida parapsilosis (21.5%), and Candida tropicalis (18.5%), and the overall non-susceptibility rate to fluconazole was 38.5%. Among 62 patients with Candida bloodstream infection 34 died, and the mortality rate was 54.8%; Univariate analysis showed that admission to intensive care unit, mechanical ventilation, combined lung infection, severe hypoalbuminemia, and no antifungal therapy within 48 hours after positive blood culture were associated with mortality (P<0.05). Logistic regression analysis showed that severe hypoalbuminemia(OR=7.924,95% CI 1.851~34.599,P=0.006) and failure to initiate antifungal therapy within 48 hours after the initial blood culture report(OR=6.828,95% CI 1.377~33.852,P=0.019) were independent risk factors for death. Non-albicans Candida infection was the main, and severe hypoalbuminemia was an independent risk factor for death in patients with Candida bloodstream infection. Early antifungal therapy was important for patient prognosis. Conclusion Candida bloodstream infection patients were characterized by multiple comorbidities and high mortality.

    A 44-year-old male farmer presented with erythematous plaques of the whole body, diffusely infiltrated erosions on his left leg and fever. Smooth, creamy-white, yeast-like colonies grew from skin biopsy specimen and blood specimen after 72 h of incubation. Electron microscopy demonstrated 'morula' or daisy-like appearance of its endosporulating sporangia. These findings were consistent with a diagnosis of protothecosis. The patient was treated with fluconazole and itraconazole but finally died. This case and the literature review will increase the knowledge of this pathogen.

    Objective To analyze the clinical characteristics and pathogen distribution of 124 cases of otomycosis in Guangzhou. Methods The clinical data of one hundred and twenty-four patients diagnosed with otomycosis and positive fungal cultures from the otorhinolaryngology department of Guangzhou First People's Hospital between January 2020 to January 2022 were retrospectively analyzed. Results The 124 patients were mainly 18-40 years old (78/124, 62.9%). Unclean ear picking (55/124, 44.4%) and frequent use of earphones (45/124, 36.3%) were the main factors for causing otomycosis. The most common clinical feature was pruritus (96/124, 77.4%), followed by fluid/pus, hearing loss, pain and tinnitus. A total of 94 strains of Aspergillus (94/124, 75.81%) were isolated, including 37 strains of Aspergillus terreus (29.8%) and 34 strains of Aspergillus niger(27.4%). Moreover, 19 strains of Candida (15.3%), 5 strains of Penicillium (4.0%), 5 strains of Malassezia (4.0%) and 1 strain of Trichophyton rubrum (0.8%) were isolated. Unclean ear picking was positively correlated with Aspergillus infection (P<0.05), hearing loss and pain were positively correlated with Aspergillus infection (P<0.05), and ear canal pruritus was positively correlated with Candida infection (P<0.05). Conclusion Aspergillus and Candida were the main fungal pathogens of otomycosis, and Malassezia was the opportunistic pathogen of otomycosis. There was an association of clinical features with pathogen distribution pattern,which should be caused by clinical attention.

    Clinical value of rapid on site evaluation in the diagnosis of pulmonary cryptococcosis

    Objective To explore the accuracy and timeliness of rapid on-site evaluation site (ROSE) in the diagnosis of pulmonary cryptococcosis by percutaneous lung biopsy. Methods A retrospective analysis of 25 cases of percutaneous lung biopsy pathologically diagnosed as pulmonary cryptococcus, including patient imaging, ROSE and pathological diagnosis results, time-consuming, and diagnosis costs was performed. Results Based on the final diagnosis, and the accuracy, sensitivity, and specificity of ROSE were 80%, 80%, and 100%, respectively. The average time for the issuance of ROSE and pathology reports were (10±1) min and (81±1.4) h, respectively; the diagnosis cost were 220 and 450 yuan, respectively. Conclusion ROSE had high accuracy and timeliness in the diagnosis of Cryptococcus neoformans pneumonia, and economic cost savings. It is worthy of application and promotion in the diagnosis of cryptococcus neoformans pneumonia in respiratory intervention.

    Objective To analyze epidemiological trends and clinical characteristics of cases caused by Kodamaea ohmeri in Asia, and to provide a scientific basis for improving the clinical diagnosis and treatment of K. ohmeri infections. Methods Using the methods of literature review and meta-analysis to analyze retrospectively the information of patient’s nationality, sex and aged composition, risk factors, onset organs, clinical manifestations, therapeutic methods and prognosis of K. ohmeri. Results K. ohmeri infections could occur in both immunocompetent and immunocompromised person. The gender ratio of the infection host was 1.59:1, predominantly in premature neonate and middle aged people, and the blood system was the most commonly affected site. More than half of the patients had immunodeficiency diseases or other risk factors, and there were no early specific symptom with chief complaints of fever. Diagnosis of K. ohmeri infections mainly relied on culture. The patients had a poor prognosis, with high mortality. Conclusion The incidence of K. ohmeri infections has increased recently and the diagnosis mainly depends on culture. Strengthening the in vitro antifungal susceptibility monitoring and the studying of epidemiology for K. ohmeri are significant to improve the diagnosis and prognosis of K. ohmeri infections.

    A 39-year-old man was found to have reduced blood cells during physical examination 8 years ago. Cyclosporine and prednisone were taken for a long time, after undergoing bone marrow, flow cytometry and other examinations to diagnose paroxysmal nocturnal hemoglobinuria (PNH). More than 2 months ago,his left lower extremity was scratched, accompanied by ulceration, redness, swelling and pain. Recently he had been admitted to the hospital and immune indicators were reduced, accompanying by secondary hyperglycemia. Wound tissues and secretions were taken for pathological examination and pathogenic testing. Piperacillin/tazobactam was used for empirical anti-infective therapy. After being diagnosed with Prototheca wickerhamii infection, the antibiotic was replaced by amphotericin B according to antimicrobial susceptibility testing. The dosage of the 1st day was 1 mg, the 2nd to 4th days were 10mg and 5th-10th days were 25 mg once a day. Due to elevated creatinine,the dosage was reduced to 15 mg every other day and wound care was performed at the same time. At last, there was no obvious inflammatory secretion and the wound had healed, when re-examining one month later.

    Diagnosis of a girl with indistinguishable tinea of vellum hair by dermatoscopy

    A 6-year-old girl presented with a light erythema on her right cheek with no scales or pustules. Under dermoscopy, the diseased hair was found to be shortened with a few scales on the root. By fluorescent staining, the diseased hair was surrounded by transparent hyphae which emitted bright blue fluorescence, and the hyphae grew along the direction of the hair shaft, some of which were extended to the surroundings. No hyphae or spores were found in the dandruff under the dermoscopy. The pathogen was identified by morphology and molecular biology as Athroderma otae (teleomorph of Microsporum canis). The diagnosis of tinea of vellus hair on a child’s face was made. Oral itraconazole capsule 100 mg/d and topical terbinafine cream were administrated. After 4 weeks’ follow-up, the child’s facial background color returned to normal, and microscopic examination showed negative result, so the medications were stopped. There was no recurrence in 3 months’ follow-up.

    A 10-year-old male patient presented with erythema,pustules and alopecia on the scalp with pain for more than 1 week. The fungal microscopic examination was positive, and Trichophyton mentagrophytes was suggested by fungal culture, The diagnosis of kerion was made.After being given terbinafine tablets combined with itraconazole capsules for antifungal and methylprednisolone anti-inflammatory treatment, the skin lesions were healed without scars, and the hair growth was normal.

    Characteristics and risk factors of fungal infection after urinary surgery

    Objective To investigate the characteristics and risk factors of postoperative fungal infection in urinary system. Methods The clinical data of 762 patients with urinary system diseases who underwent urological surgery in our hospital from June 2017 to may 2021 were retrospectively analyzed. According to whether they had urinary system and blood fungal infection after operation, they were divided into infection group (n=75) and non infection group (n=687). The clinical data of the two groups were compared, and univariate analysis and logistic regression analysis were performed. Results 75 of the 762 patients had postoperative fungal infection, the incidence was 9.84%. The main disease type was urinary calculi, the main pathogen was Candida, and the main indwelling catheters were DJ tube and catheter; Among 75 cases of fungal infection after urinary system operation, 69 cases were asymptomatic, 5 cases had frequent urination, urgency and pain, and 1 case had chills, fever and low back pain. The results of univariate analysis showed that the proportion of age >65 years, open operation, operation time ≥ 30 min, drainage tube retention time >7 d, antibacterial drug use time ≥ 3 d in the infection group was higher than that in the non infection group (P<0.05), and the blood glucose level and CRP level in the infection group were higher than those in the non infection group (P<0.05); Logistic multivariate analysis showed that age (OR=1.604), operation method (OR=1.139), catheter retention time (OR=2.537), blood glucose level (OR=2.753), antibacterial drug use time (OR=2.508) and CRP (OR=3.057) were independent risk factors for fungal infection after urinary surgery (P<0.05). Conclusion The common pathogen of postoperative fungal infection in urinary system was Candida and most of them had no clinical symptoms. The common disease was urinary calculi. The common catheters indwelling were DJ tube and catheter. Age >65 years old, open surgery, catheter retention time >7 d, elevated blood glucose level, more than 3 d use of antibiotics, and elevated CRP were independent risk factors for postoperative urinary fungal infection, which need targeted intervention.

    Objective The aim of this study is to the clinical features of fungal spheroid fungal sinusitis and osteitis. Methods The clinical data of 105 FBS patients (110 sinuses in total) were retrospectively analyzed,the clinical features of their symptoms, signs, image, pathology, and osteitis were summarized and analyzed. Results Among the 110 sinuses, 97 sinuses (88.2%) were located in the maxillary sinus, and 13 sinuses (11.8%) were located in the sphenoid sinus; 97 sinuses (88.2%) had different degrees of bone thickening compared with the contralateral sinus. The wall bone of lesion sinus and contralateral sinus were significantly different in bone thickness (0.34±0.17 vs. 0.14±0.08), CT value (1161.5±322.5 vs. 864.2±482.8), GOSS score (3.0±1.4 vs. 0.3±0.9) .Pathological observation showed manifestations of osteomyelitis accompanied by bone remodeling activities. Conclusion Fungal spheroid fungal sinusitis prefer to occurs in the maxillary sinus and sphenoid sinus, mainly in single sinus. Most of the wall bone around the lesion sinuses have obvious osteitis, which is mainly characterized by bone thickening and increased CT value.

    Objective To investigate the clinical distribution of 94 patients with blood flow infection of Candida and the related risk factors of death. Methods The clinical characteristics and death risk factors of patients with blood flow infection of Candida diagnosed in Liuzhou People's Hospital from January 2019 to December 2021 were analyzed retrospectively. Results From 2019 to 2021, 94 cases of Candida were detected in blood flow infection patients, mainly distributed in ICU and surgery, accounting for 58.5%; The top three Candida species were C. albicans, C. tropicalis and C. parapsilosis, accounting for 48.9%, 16.0% and 13.8% respectively;33 of 94 patients with blood flow infection of Candida died, the mortality rate was 35.1%. C. albicans (57.6%, 19/33) was most frequently detected in the dead patients, mainly distributed in ICU (57.6%, 19/33). The sensitivity rate of C. parapsilosis to azoles was 100%, the resistance rate of C. albicans to triazoles was less than 7% and the resistance rate of C. glabrata and C. tropicalis was 20%~26.7%. Univariate analysis showed that age ≥60 years, ≥2 basic diseases, mechanical ventilation and endotracheal intubation were associated with mortality (P<0.05). Logistic binary regression analysis showed that mechanical ventilation was an independent risk factor associated with mortality in patients with Candidal blood flow infection. Conclusion The detection rate of C. albicans in Candidal blood flow infection was the highest. Mechanical ventilation was an independent risk factor for death in patients with blood flow infection of Candida.

    A 62-year-old women came to our hospital because of nodules on the right hands for six months, nodules on the right arms for three months. A diagnosis of sporotrichosis was made based on typical manifestations, including positive fungal culture, histopathological and genetic examinations. Daily oral itraconazole combined with fractional laser and photodynamic therapy three times, that case was cured after one month.

    Periprosthetic fungal infection was a catastrophic complication after artificial joint replacement, and its diagnosis and treatment had always been one of the hot research topics in the field of orthopedics. A case of a 66-year-old immunocompetent man who underwent total knee arthroplasty and infected with Candida glabrata six years later with signs of prosthetic infection were reported. The pathogenic diagnosis and anti-fungal therapy might provide a reference for the drug treatment of periprosthetic fungal infection after artificial joint replacement.

    Kerion is a fungal skin disease caused by the infection of Microspore genus,which can lead to a large area of cicatricial alopecia and affect the physical and mental health of patients. Early correct diagnosis is extremely important. Fungal pathogen detection is the main basis for the diagnosis of tinea capitis at present, but there is a certain false negative rate in fungal direct microscopic examination, and it takes a long time for fungal culture. One case of kerion in a child was diagnosed by dermatoscopy and fungal fluorescence staining. Fluconazole combined with glucocorticoid treatment was satisfactory.

    Objective To determine the expression level of EREG in the blood-brain barrier infected by Cryptococcus neoformans and to explore the role of EREG crossing the blood-brain barrier. Methods To construct the model of blood brain barrier in vitro by using Cryptococcus neoformans and Transwell chamber, and PBS was used as the control group. According to the time of Cryptococcus infection, divided into three groups: 3 h, 6 h and 12 h. The changes of the adhesion and penetration ability of Cryptococcus to the blood-brain barrier were detected by detecting the transmembrane resistance of single cells in the upper chamber of the Transwell and the suspension coating in the lower chamber of the Transwell at different time of infection, and the expression level of EREG in vascular endothelial cells during the process was detected. Results The expression of EREG was significantly increased and positively correlated with the time of infection during the course of cryptococcal infection with microvascular endothelial cells. Conclusion These results indicated that the ability of Cryptococcus to cross the blood-brain barrier was closely related to the expression level of EREG.
