106株红色毛癣菌基因分型与药敏分析

The genotyping of Trichophyton rubrum and the analysis of drug sensitivity

摘要: 目的 探索海南地区的红色毛癣菌基因型与感染部位、药敏的关系。方法 基于红色毛癣菌核糖体rDNA非转录区(NTS)的基因分型进行种内分型,依据"CLSI-M38-A2"方案进行药敏实验。结果 106株红色毛癣菌TRS-1基因型有5种带型,其中TypeⅠ52株(49.06%),TypeⅡ14株(13.21%),TypeⅢ5株(4.72%),TypeⅣ型3株(2.83%),其他带型32株(30.18%)。TRS-2基因型有3种带型,其中TypeⅠ68株(64.15%),TypeⅡ6株(5.66%),其他带型32株(30.19%)。药敏结果MIC几何均数由低至高分别为特比萘芬(0.0092 μg/mL)、伏立康唑(0.0181 μg/mL)、伊曲康唑(0.1491 μg/mL)、酮康唑(0.1630 μg/mL)、氟康唑(2.3164 μg/mL)。有42株菌表现出对抗真菌药物不敏感,不敏感菌株TRS-1和TRS-2分型均以TypeⅠ为主。结论 海南地区流行的红色毛癣菌TRS-1和TRS-2基因型均以TypeⅠ为主。以对特比萘芬(0.0092 μg/mL)、伏立康唑(0.0181 μg/mL) MIC几何均数最低。本结果发现本地区的红色毛癣菌不敏感菌株与基因带型关系不大,也与感染部位无关,可能与来源有关。

Abstract: Objective To study the relationship between the genotype of Trichophyton rubrum and the site of infection and drug sensitivity in the area. Methods Perform intraspecies typing based on the genotyping of the ribosomal rDNA non-transcribed region (NTS) of Trichophyton rubrum, and test the drug sensitivity according to the "CLSI-M38-A2" protocol. Results There were five banding types in 106 Trichophyton rubrum TRS-1 genotypes, including TypeⅠ52 (49.06%), TypeⅡ14 (13.21%), TypeⅢ5 (4.72%), TypeⅣ3 (2.83%), and others 32 strains (30.18%). There are three banding types in TRS-2 genotypes, including TypeⅠ68 strains (64.15%), TypeⅡ6 strains (5.66%), and others 32 strains (30.19%). The geometric mean MICs of drug sensitivity results from low to high were terbinafine (0.0092 μg/mL), voriconazole (0.0181μg/m), itraconazole (0.1491 μg/mL), ketoconazole (0.1630 μg/mL), and fluconazole (2.3164 μg/mL). 42 strains showed insensitivity to antifungal drugs, and the insensitive strains TRS-1 and TRS-2 were mainly type I. Conclusion Both the TRS-1 and TRS-2 genotypes of Trichophyton rubrum in Hainan are mainly TypeⅠ. Terbinafine (0.0092 μg/mL) and voriconazole (0.0181 μg/mL) have the lowest geometric mean MICs. This result found that the insensitive strains in this area have little relationship with the red hair genotype, and have nothing to do with the infection site, and may be related to the source.
